Medical Forms

The Medical Form is to be printed off, completed and returned to the appropriate Contact Scouter. This form is a mandatory form that is to be handed in at the start of the Scouting year prior to commencement of any activities. This form is to be completed for ALL youth as well as adults that are planning to volunteer.
